Any person taking > 5 medications should have a GP, Clinical Pharmacy or Specialist Nurse medications review.

This should be done annually or if there is a change in circumstances and /or any deterioration.

Incontinence should be taken seriously and requires an assessment to manage or prevent.

Please complete a Comprehensive Geriatric Assessment (CGA) and follow local continence pathways.

This is support for people who are in the last months or years of their life it should help them to live as well as possible until they die, and to die with dignity.

Dementia: begins slowly and is gradually noticed over time and cannot be reversed.

Delirium: a sudden onset and can usually be reversed.

Both require further assessment and a proper diagnosis.

Establish a diagnosis. Follow local delirium pathway.

Any fall should be taken seriously as there is no such thing as simple fall.

Please complete a PRISMA or an Edmonton if someone has a fall. Also please complete a Comprehensive Geriatric Assessment (CGA) and refer on appropriately.
